This
new council "Training Attendance Report" form
is an improved and digital version to replace the old
three-part carbonless national form (#34413). It now
includes a "District" column to track district
information for all attendees and, in this digital
format, is easy to fill out and submit via e-mail or
fax.
Please start using this new form for all future training
courses. Your prompt submission to the council will help
us in our new initiative to collect accurate "trained" date
for all adult volunteers. Customized versions are available
below for most basic training courses, in two formats:
